EVALUATION OF THE PELVIC-LOWER LIMB ALIGNMENT USING STER EOS -THE COMP

Description

Objectives: ster EOS is focused as a low-dose imaging tool for the three-dimensional evaluation of standing-weighted whole-body alignment. This study aims to evaluate pelvic-lower limb alignment after Open wedge distal tuberosity osteotomy (OWDTO) and Total Knee Arthroplasty (TKA).

Methods: 52 patients who underwent OWDTO and TKA in our hospital were included and underwent ster EOS at pre-operatively and post-operative 6 months. The radiographic parameters including the coronal parameters (weight bearing ratio (WBL), coronal Hip knee angle (c-HKA), pelvic obliquity (PO)) and sagittal parameters (sagittal HKA (s-H KA), pelvic incidence (PI), pelvic tilt (PT) and sacral slope (SS)) were assessed. Furthermore, the relationship between the difference in parameters was analyzed.

Results: The coronal parameters except for PO were changed significantly. The coronal parameters except for PO were changed significantly in both groups. However, regarding the sagittal parameters, PI, s-HKA, and SS are changed significantly in only TKA group. (PI:55.0±8.5 vs 34.1±5.8, S-HKA: 14.0±1.4 vs 10.7±4.5°, SS: 29.7±7.0 vs 34.2±5.8, p<0.001) Additionally, the positive correlation between ΔSS and Δs-HKA was statistically significant. (r=0.49 [0.12-0.74], p=0.01)

Conclusions: TKA significantly improved the pelvic-lower limb sagittal alignment compared to OWDTO. Therefore, assessing the pelvic-lower limb alignment for knee surgery is important.
