Accuracy of surgical posterior tibial slope reconstruction in TKA

This study highlights the significant variability in preoperative posterior tibial slope (PTS) among patients undergoing total knee arthroplasty (TKA) and discusses the challenges in achieving a consistent postoperative PTS, noting that existing guidelines do not match the individual medial preoperative PTS in a majority of cases.

The impact of pretibial subcutaneous fat (PSF) and surgeon skill level on the accuracy of PTS modification during TKA was investigated, employing a standardized surgical approach for altering PTS in patients. The findings reveal that neither PSF nor surgeon skill level significantly predicts the accuracy of PTS alteration, with an overall reduction in mean postoperative PTS compared to preoperative values.

It can be concluded that precise surgical alteration of PTS in TKA is achievable, underlining the practicality of both reducing and reconstructing the PTS, independent of local soft tissue thickness or surgeon expertise.
